As a side product, had to split two other files;
- i915_gem_fence_reg.h
- i915_gem_object.h (only parts that needed immediate untanglement)
I tried to move code in as big chunks as possible, to make review
easier. i915_vma_compare was moved to a header temporarily.

The plan is to move obj->pages out from under the struct_mutex into its
own per-object lock. We need to prune any assumption of the struct_mutex
from the get_pages/put_pages backends, and to make it easier we pass
around the sg_table to operate on rather than indirectly via the obj.

The plan is to make obtaining the backing storage for the object avoid
struct_mutex (i.e. use its own locking). The first step is to update the
API so that normal users only call pin/unpin whilst working on the
backing storage.

At the moment, we have dependency on the RPM as a barrier itself in both
i915_gem_release_all_mmaps() and i915_gem_restore_fences().
i915_gem_restore_fences() is also called along !runtime pm paths, but we
can move the markup of lost fences alongside releasing the mmaps into a
common i915_gem_runtime_suspend(). This has the advantage of locating
all the tricky barrier dependencies into one location.
v2: Just mark the fence as invalid (fence->dirty) so that upon waking we
will be sure to clear the fence after use, or restore it to the correct
value before use. This makes sure that if the fence is left intact
across the sleep, we do not leave it pointing to a region of GTT for the
next unsuspecting user.

We want to decouple RPM and struct_mutex, but currently RPM has to walk
the list of bound objects and remove userspace mmapping before we
suspend (otherwise userspace may continue to access the GTT whilst it is
powered down). This currently requires the struct_mutex to walk the
bound_list, but if we move that to a separate list and lock we can take
the first step towards removing the struct_mutex.
v2: Split runtime suspend unmapping vs regular unmapping, to make the
locking (and barriers) clearer. Add the object to the userfault_list
prior to inserting the first PTE, the race between add/revoke depends
upon struct_mutex for regular unmappings and rpm for runtime-suspend.

During rpm resume we restore the fences, but we do not have the
protection of struct_mutex. This rules out updating the activity
tracking on the fences, and requires us to rely on the rpm as the
serialisation barrier instead.

What I never hit in testing, but Mika immediately did, was a GPU hang
with a pending fence release (where a tiled object has been changed by
the user to be untiled, and the update has not yet been committed to the
fence register). As the stride/tiling is 0, this causes a divide-by-zero
error when trying to write the new fence parameters:

Our current practice is to only name the actual list (here
dev_priv->fence_list) using "list", and elements upon that list are
referred to as "link". Further, the lru nature is of the list and not of
the node and including in the name does not disambiguate the link from
anything else.

By moving map-and-fenceable tracking from the object to the VMA, we gain
fine-grained tracking and the ability to track individual fences on the VMA
(subsequent patch).

If we cannot release the fence (for example if someone is inexplicably
trying to write into a tiled framebuffer that is currently pinned to the
display! *cough* kms_frontbuffer_tracking *cough*) fallback to using the
page-by-page pwrite/pread interface, rather than fail the syscall
entirely.
Since this is triggerable by the user (along pwrite) we have to remove
the WARN_ON(fence->pin_count).

Treat the VMA as the primary struct responsible for tracking bindings
into the GPU's VM. That is we want to treat the VMA returned after we
pin an object into the VM as the cookie we hold and eventually release
when unpinning. Doing so eliminates the ambiguity in pinning the object
and then searching for the relevant pin later.

In the previous commit, we moved the obj->tiling_mode out of a bitfield
and into its own integer so that we could safely use READ_ONCE(). Let us
now repair some of that damage by sharing the tiling_mode with its
companion, the fence stride.

With the introduction of requests, we amplified the number of atomic
refcounted objects we use and update every execbuffer; from none to
several references, and a set of references that need to be changed. We
also introduced interesting side-effects in the order of retiring
requests and objects.
Instead of independently tracking the last request for an object, track
the active objects for each request. The object will reside in the
buffer list of its most recent active request and so we reduce the kref
interchange to a list_move. Now retirements are entirely driven by the
request, dramatically simplifying activity tracking on the object
themselves, and removing the ambiguity between retiring objects and
retiring requests.
Furthermore with the consolidation of managing the activity tracking
centrally, we can look forward to using RCU to enable lockless lookup of
the current active requests for an object. In the future, we will be
able to query the status or wait upon rendering to an object without
even touching the struct_mutex BKL.
All told, less code, simpler and faster, and more extensible.

The future annotations will track the locking used for access to ensure
that it is always sufficient. We make the preparations now to present
the API ahead and to make sure that GCC can eliminate the unused
parameter.

In the future, we will want to add annotations to the i915_gem_active
struct. The API is thus expanded to hide direct access to the contents
of i915_gem_active and mediated instead through a number of helpers.

In the next patch, request tracking is made more generic and for that we
need a new expanded struct and to separate out the logic changes from
the mechanical churn, we split out the structure renaming into this
patch.

The existing for_each_sg_page() iterator is somewhat heavyweight, and is
limiting i915 driver performance in a few benchmarks. So here we
introduce somewhat lighter weight iterators, primarily for use with GEM
objects or other case where we need only deal with whole aligned pages.
Unlike the old iterator, the new iterators use an internal state
structure which is not intended to be accessed by the caller; instead
each takes as a parameter an output variable which is set before each
iteration. This makes them particularly simple to use :)
One of the new iterators provides the caller with the DMA address of
each page in turn; the other provides the 'struct page' pointer required
by many memory management operations.
Various uses of for_each_sg_page() are then converted to the new macros.

We have varied reports of swizzling corruption on gen4 desktop, and
confirmation that one at least is triggered by uneven memory banks
(L-shaped memory). The implication is that the swizzling varies between
the paired channels and the remainder of memory on the single channel. As
the object then has unpredictable swizzling (it will vary depending on
exact page allocation and may even change during the object's lifetime as
the pages are replaced), we have to report to userspace that the swizzling
is unknown.
However, some existing userspace is buggy when it meets an unknown
swizzling configuration and so we need to tell another white lie and
mark the swizzling as NONE but report it as UNKNOWN through the extended
get-tiling-ioctl. See

drm/i915: Declare the swizzling unknown for L-shaped configurations
for the previous example where we found that telling the truth to
userspace just ends up in a world of hurt.
Also since we don't truly know what the swizzling is on the pages, we
need to keep them pinned to prevent swapping as the reports also
suggest that some gen4 devices have previously undetected bit17
swizzling.
v2: Combine unknown + quirk patches to prevent userspace ever seeing
unknown swizzling through the normal get-tiling-ioctl. Also use the same
path for the existing uneven bank detection for mobile gen4.

Make I915_READ and I915_WRITE more type safe by wrapping the register
offset in a struct. This should eliminate most of the fumbles we've had
with misplaced parens.
This only takes care of normal mmio registers. We could extend the idea
to other register types and define each with its own struct. That way
you wouldn't be able to accidentally pass the wrong thing to a specific
register access function.
The gpio_reg setup is probably the ugliest thing left. But I figure I'd
just leave it for now, and wait for some divine inspiration to strike
before making it nice.
As for the generated code, it's actually a bit better sometimes. Eg.
looking at i915_irq_handler(), we can see the following change:

Similar to commit c44ef60e437019b8ca1dab8b4d2e8761fd4ce1e9 ("drm/i915/gtt:
Allow >= 4GB sizes for vm"), i915_gem_obj_offset and i915_gem_obj_ggtt_offset
return an unsigned long, which in only 4-bytes long in 32-bit kernels.
Change return type (and other related offset variables) to u64.
Since Global GTT is always limited to 4GB, this change would not be required
in i915_gem_obj_ggtt_offset, but this is done for consistency.
v2: Remove unnecessary offset variable in do_pin, as we already have
vma->node.start (Chris).
Update GGTT offset too (Tvrtko).

Chris rightfully suggested that documenting fences without documenting
the BO tiling tracking doesn't make much sense, so fix that.
The important bit to stress here (since it lead to some confusion) is
the GEM doesn't really care about tiling. Except for a few select cases
where the kernel needs to manage something that userspace can't take
care of: Namely the limited number of fences and fixing up swizzling,
although we still fail at the later.
v2: Move the low-level tiling/swizzling functions and kerneldoc to
i915_gem_fence.c and leave only the userspace interface here.
