tet: Add change notifications to comply with the annoying terms of the 'Artistic' license.

From the license:
       3. You may otherwise modify your copy of this Package in any way,
       provided that you **insert a prominent notice in each changed file
       stating how and when you changed that file**, and provided that you do
       at least ONE of the following:

Most, but not all, of the files in src/tet3 do not have the X11 license at the
top and so are covered by this clause.
